Q: What information is listed
both on the health insurance ID card? Is the insured's name, address,
SS number, date of birth, etc. listed on the e-mailed or permanent ID
card?
A: For privacy reasons, most of
this personal information is not included on a health insurance ID
card. A health insurance ID card usually contains the name of the
insurance company or plan administrator, the policy number and
effective date of coverage, the member services telephone number and
the name of the policyholder. The main purpose of the ID card is to
allow medical service providers to verify coverage and obtain billing
information.
